This exercise will be used with incompetence in the third third of the tongue, of the oral diaphragm and the pharynx musculature (for example with the K-sound in snoring, sleep-apnoea and swallowing difficulties). It is conceived for the third training phase after having completed the exercises for the first and second third of the tongue. The target will be to define the enlarged frame of mobility by touch as well, as the strengthening of the muscles of the root of the tongue, oral diaphragm and pharynx. . Specific motion and strengthening exercise of a circumscribed muscle group (see above) with the aid of surface sensitivity. |

Materials: |

Cornflake, grain flake. |

Procedure: |
|
- Previous exercises: During swallow training: The exercises for the first and second tongue third. For accompanying exercises see paragraph “Discussion”. |

-Starting position: Recommendable the ,L-M’-POSITION. |
|
- Step-by-step-description: -1 The patient is asked to say ,KICK’ several times while trying to feel what the back end of his tongue does. Does he feel its surface touching the palate? This is to be practiced. (For self control and to position the flake a hand mirror may be used.) -2 At first the therapist, later on the patient himself will position a flake on the back tongue end (middle of the last tongue third). -3 Say ,KICK’ and feel the contact. -4 To increase the exercise intensity the patients tongue tip may be held on to with two fingers, mouth opened, repeating ,KICK’. -Timing: The flake will only be “compatible” about for 10 repetitions which means a rate of 10 ,KICK’ at second intervals to one sequence. With a new flake the sequence will be repeated, may be to make a threefold series. |

Remarks: |
With a tendency to retch the exercise should be adapted gently through putting the flake so far to the front, first, that no reflex is triggered, later on pushing it further and further back until the desired site is reached. |
Discussion: |
As a completive exercise the WORDEXERCISES 3. THIRD may be set. With the exercise SQUEEZE-A-FLAKE, a hold-exercise, the perseverance of the respective muscles will get optimized, with the ELEVATOR its mobility while in the exercise discussed here the feel training has priority. The tongue will learn to feel what its back end is doing. When you say ,KICK’ it makes quite a long cats hunchback from the tip at the POINT till back in the throat. To monitor this the tongue gets a cornflake positioned piggyback on its back end, there, where we have practiced it together. In the beginning you should precisely control it with a hand-mirror. |
