This exercise will be used in case of deficiencies in mobility or  motility and for desorientations in the Orbicularis region. Furthermore it may be carried out in the manner of a UNILATERAL EXERCISE. (Mainly Anatomy I/1, page 3).

Contents:

Included in the training process is the maneuverability of the muscle tracks leading radially into the Orbicularis. In the carry-out the unilateral variation is playing a special role as with this method the patient may be motivated to imitate the  maneuverability of the “healthy” side with the “ill” side. The therapeutic effect will be (self-)monitored visually.

Materials:

Toothpick, wooden spike or thin drinking straw.

Procedure:

- Step-by-step-Description:

-1 The wooden spike is grasped with the lips.

-2 The patient tries to move the stick in a manner like a conductor directing an orchestra with a baton.

For that several different movements may be prescribed: in circles or up and down, to and fro, doing a triangle and the like. In prescribing of the precise motion the dysfunction will be expressively taken into account.

If there is especially one side to be trained the stick is caught unilaterally. About this issue see the description in the catalogue under chapter UNILATERAL EXERCISE.

- Timing:

 The chronological process is not specified.  After having been instructed the patient should carry out the exercise several tinmes over the day.

Characteristics:

Mobility, motility, fine motor skills, tone rising.

Remarks:

Assuming the  motor skills having disappeared completely on one  mouth corner side this  would just the condition fitting the project aim. The healthy side will, now, demonstrate how the movement should run followed by the disabled side trying to imitate it as correctly as possible. At first there may only be the will, the thought to act. With an insistend training already the slightest flutter of the tip of the stick will mean a success; This is the reason why the  little stick is used, to accentuate the movement making it visible.

For this item see also the “Discussion” and description in chapter UNILATERAL EXERCISE. That exercise is especially matched on functional deficits in the perioral musculature. It belongs to a group of exercises which aim at a substantive section for the given situation. With this exercise the multiple  different fibre tracks are targeted collectively. Therefore will be required a precise prescribing of the CONDUCTING, of the process of the movements being carried out as well as their monitoring, at first by the therapist, later on – using the hand-mirror – by the patient himself. The repeatedly mentioned difficulty to conclusively correlate the exercises to a defined target musculature is likewise implemented in this exercise. CONDUCTOR is targeting presumably the radial fibre tracks;. In case the circular muscle fibres are also reacting incompetently the work out will, of course, not be viable as they are not able to hold the stick.

Instructions:

A good help for the movability of the mouth is the buton, the instrument of the conductor.

A toothpick is held by the lips. With this stick, now, an orchestra is going to be conducted. Of course you may switch on some music for this and start conducting vivaciously. It will be of great use if you post yourself in front of a mirror. By the way, even if the movements are so tiny that you would need a magnifying glass to spot them, don't mind, skill comes with practice, even with the conductor's buton, even at the podium.

In case one side does not want to join the play just let the healthy side lead the way. Then the disabled side will follow in as well as can be to imitate the  movements. Always alternately one and the other side and frequently!

If it works well in principle then, later on, a particular movement is to be practiced, up and down, to and fro, in circles or in a triangle.
