 SQUEEZE-A-FLAKE

The exercise will be used in case of an incompetence in the third tongue-third and parts of the mout-hfloor and pharyngs muscles as with a faulty articulation of the K-sound, with snoring or sleep-apnoe, swallowing disturbance in the third training phase – after having completed the exercises for the first and second tongue-third. The training objective is the full length orientation of the tongue towards the palate in its standby position through strengthening of the extrinsic musculature especially the Styloglossus [018], muscles of the tongue ground, mouth floor [023]and throat  [026 / 028]as well as a dynamic pressure upwards in the rear area during the swallowing reflex. Special target musculature: M. styloglossus.

Contents:

Physiotherapeutic classifications as dynamic, kinetic, static, habitualizing, coarse or fine motoric.

Materials:

Facultatively for the start elastic and hand-mirror for self-control.

Procedure:

- Previous exercises – for the swallow training are the exercises for the first and second tongue third. As for complementary exercises see paragraph “Discussion”.

-Starting position:

The mouth is held wide open, the tongue tip looks out without touching its surroundings.

- Step-by-step-Description:

-1 Next the therapist, later on the patient himselfe, positions a flake on the posterior tongue end (approximately the middle of the last tongue third). To check the positioning of the flake a hand mirror may be used.
-2 In the early stage the therapist taps on the very place on the palate with the end of a spatula where the flake is going to be pressed against.
The tongue , now, ascends and squeezes the flake firmly against this point. Count to ten and back to the start.
To choice only the last tongue end may be lifted up or additionally the tongue tip to the SPOT.
As an intensification of the training intensity the position may be held under a stepwise prolongation of the counting time.
For an increase especially with a tendency for a retral tongue broadening position the tongue may be tried to keep as slender as possible ( with an increasing mouth opening)..
Furthermore the pressure and the smallness may be carried out with maximum energy but this only with counting to ten as under these conditions a strong twinge may occur.
-Timing: Counting to ten in one sequence may be added up to a series of ten sequences either the flake may be held until dissolving.

Characteristics:

Well-directed hold- and strengthening exercise of a defined muscle group ( see before).

Remarks:

In case of a tendency to choke in reflex the exercise should be settled in cautiously by positioning the flake at first as far at the front as not to trigger the reflex and later on more and more pushing it backwards into the desired site.

Discussion:

A supplementary exercise would be the WORDEXERCISES 3. THIRD. The here discussed exercise SQUEEZE-A-FLAKE strengthens the perseverance of the respective musculature to gain  the required frame of motion. A similar exercise is the ELEVATOR which aims at mobility while in the KICK exercise the capacity of feeling (esthesis, kinesthesis) is emphasized. For further exercises dealing with this target field see BACK-OF-MOUTH-EXERCISES. In our paragraph “Characterization” the exercise furthermore got the rating ‘Pseudomanipulation’ as a device (foreign body, material, equipment) is utilized which, by appointment, means an intervention, a manual/physical activity (=Manipulation) during the work-out. The manipulation practised here already almost fits into the frame of  physiological demands as foodstuffs on the back of the tongue might not be seen as unnatural.

Instructions:

The tongue will have to learn to get up with its back end. She should know how if you want to swallow correctly.

Thus you, at first, do as if going to slurp and swallow hot tea. With the ‘gulp’ you should precisely feel where the tongue far at its end presses against the palate with its back.

This is precisely the point we have concentrated on in the session. Back there, now, a cornflake ( or something similar) is positioned upon the tongue back. The tongue lifts the flake upand sweezes it against the roof.

This will be continued until it dissolves into flake mash which you may, later on, swallow in the way of the ,1-2-3’-EXERCISE. Else the exercise is repeated as often as we agreed on.
