This exercise is brought to action for setting up the initial phase of the swallow reflex chain, i.e. the moulding of the dimple and especially its marginal sealing by the tongue (dimple) margin when the fine motor motility is impaired. (See also SWALLOW-MECHANISM). ((Anatomy: I.3.5,Page 25a.) | |

Procedure: |

- Previous exercises: Generally all necessary exercises previously to entering the progressive phase of the reflex exercises- directly: DIMPLE, WATERCARRIWER: |

-Starting position: No specification, recommendable BASIC-POSITION: |

- Step-by-step-description: -1 Previously one well done swallow. The mouth opens and the tongue shows up forming the DIMPLEon its first third. -2 Primarily the therapist, later on an assisting person or the patient himself fills half a teaspoon of water into the dimple. -3 The loaded dimple, as in WATERCARRIWER , is shifted up and backwards, held with the tongue-tip to the POINT and pressed tightly to it. -4 With the mouth wide open the head is now tilted forward with a jolt face towards the ground. No water is allowed to seep from the dimple = the dimple has to form a WTAER RETAINER. -5 …and swallow, best after a rule like the ,1-2-3’-EXERCISE. |

Discussion: |
Besides the motivating game effect this exercise is especially suitable as a monitoring for WATERCARRIWER and pre-exercise for the LIQUID SWALLOW. By this procedure the formerly by the MFT taught “tube squeeze” (this was to be a comparison to the supposed course of the swallow reflex “similar to the squeezing out of a tube of tooth paste”) as a crude motor operative swallow pattern is exceeded by the coordinatively higher ranking and functionally more efficient training of the distal movement of the dimple-formed retraction (dimpling) of the tongue surface.
With the, in contrary to former times, technically much more precise examination possibilities especially in this field the, with the unreinforced eye scarcely noticeable, fine motor coordinated movement courses permit the gaining of much more detailed insights.
If an increase of the training efficiency is desired the exercise may be modified from step 4 on in a way, that after the counting to ten the head is moved in the upright position again, counted to ten again, tilted forward, water retained counting, upright again and so forth. Here a quite interesting variation may be cited in which the work-out is carried on after step 4 standing on ones head while doing the swallow. Quite a varying alternative go off alright. |

This time the tongue may enjoy a game. Here it may proof if it has learned the DIMPLE correctly. The mouth gets open and the tongue look out having opened up the DIMPLE on its front. In there we drip some drops of water. These are carried by the tongue to the POINT where it holds the dimple tightly pressed to the palate because, now, the head is tilted far forward with the mouth open. And? There will no water be allowed to run from the dimple! Show it! And repeat as often, as we came about. |
