The exercise will be brought into action with a hyperactive Mentalis for a controlled training of the downward shift and position of the chin soft tissue (see also ‘Phases’ in the paragraph “Discussion”). (Anatomy: Areal I, 12, Seite 09).

Contents:

Mobility exercise for mainly the Mentalis and the chin region.

As a start support the exercise may be carried out with the manipulatory help of a finger (see below).

Materials:

Tooth prick for the optional second phase.

Procedure:

- Previous exercises: Advisable would be the BASIC-POSITION.

-Starting position: ,M’-POSITION

- Step-by-step-description:

-1 The lower lip is moved down slowly and under hand mirror control revealing the lower incisors. As an illustration the patient gets the instruction to draw a face as if finding something disgusting and therefore uttering an ‘Eeeeeeee’. With this besides the Platysma mainly the Depressor labii inferioris (Anatomy S.18) gets tensed. The meaning is to let the Mentalis

loose to facilitate the downward movement.

-1 a In case of the frequently occurring onset difficulties a starting aid may be added: The patient’s index finger is set on the chin tip pressing it softly down in the direction of the desired position. This helping hand should be omitted as soon as possible.

-2 a In the hold-mode the end position is held under counting .

-2b In the kinetic mode after attaining the end position immediately the start position is regained, then the end position, start position and so forth in a steady change counting on.

(-3 For an ALTERNATIVE EXERCISE  the BABBLE might be useful for loosening or else a contrasting movement as for instance with DUCKBILL, TIED-UP SACK or the like.)

-Timing: In step 2a the sequence is filled up by holding while counting, for example to ten and increasing. In 2b the sequence gets frequency beat in an up and down by the second. To choice three to five sequences may be put in a row or performed as an alternating sequence as described under 3.

Characteristics:

Mobility, orientation, motivation for self monitoring, stand-by position, stretching ( in 1a manipulated), gross motor, under 2a static, under 2b kinetic, tone reducing.

Remarks:

A Mentalis hyperactivity often is quite therapy resistant and demands a frequent, conscientious and controlled training.

Discussion:

In the first place this exercise aims at the stretching (distending and loosening) of a hyperactive  - mostly continually, frequently only when swallowing- [Grimacing Swallow] – upwards pushing Mentalis.

Finally a relaxed mouth slit closure should be reached without the Mentalis being involved in shuffling up the soft tissues of the lower lip region. (The Rima Oris should not close passively by this shuffling up but actively through contracting the circular Orbicularis tracks.)

In the first and main training phase it is necessary to counteract the shuffling up by aimed downwards movements. In a subsequent second phase it should be tried to stretch the tissues between chin tip and lower lip rim in a similar motion sequence but under ensuring the closure of the mouth slit, i.e. without baring the teeth. This, hence, is a matter of combining a mobility and a stretching exercise (see also the ‘Face of Aging’ and ,MOI’-EXERCISE).

In this phase a tooth pick may be used as an aid which is held loosely between the lips und thus guarantees the relaxed closure of the Rima Oris.

Instructions:

The chin to goes lumpy.

In a normal face the chin does not try to always try to pay the nose a visit – what really looks far better!

Aside from that the lumping harms the jaw and the teeth.

How do we break the chin of the lumping?

At first take up the ,L-M’-POSITION which means the tongue to the SPOT and the lips slackly together. And now:

If you find something disgusting you say:”Eeeee e e!”

And you bare your teeth, but not like when smiling, this time more the lower ones; the lower lip is backing down. Precisely this it has to do now.

So: Say ‘Eeee’ and bare the lower teeth, hold it and count to ten. If you can master this, now, comes the catchier part: Let the lips stay closed, say an ‘Eeee’ again, count to ten and so on. Ten times in a row or as often as agreed.
