HOLD-AND-PULL

Application:

This exercise is useful for frontal tongue problems coupled to the desorientation of the tongue-tip and missing starting position (POINTPOSTURE) whereas besides the neuromotor movement restriction the textural, too, is considered. Therefore this exercise rather belongs to the texture stretch wor-outs; the musculature aimed at lies more in the region of the extrinsic ztongue muscles (M. Styloglossus) (Anatomy, 1.2, page 18ff ) and for the tender shape in the intrinsic transversal tracks.

Contents:

The keeping of a starting position is combined with a STRETCHING EXERCISE urging the hold strain.

Materials:

Not required.

Procedure:

- Previous exercises: The POINT  should have been introduced the exercises for it preferably passed. As a preliminary for this exercise in case of a textural mobility restriction the SUCK-AND-PULL exercise is recommendable where the stretching quality is prevailing. For a basis, moreover, the exercises  TONGUE-PLOP and  FOZEN PLOP may be useful which can be prescribed as a direct preliminary exercise.

-Starting position: The SUCK-AND-PULL exercise wich will, then, be followed immediately by:

-1 when “air is let in” while the tongue remains in the starting position without sucking fast to the roof – tip at the POINT body in hunched position of the  CAT’S HUNCHED BACK .

Later on this will make the position to begin with.

-2 The tongue (tip) concentrates on holding the position when the mandible opens constantly more and more till reaching the maximum position, i.e. when the tongue is about to let go.

This position will be held while counting.

Breathing goes on through the mouth to avoid a covert being sucked tight to the roof.

The training success is measured by recording the maximum obtainable incisal edge distance of the front teeth.

 -Timing: A slow and powerful work-out is requested. In accordance to the training mode lengthy sequences evolve. An increase of the training mode will be obtained as well through the opening width as with the counting intervals. Recommendable is counting to, at first, twenty, later on increasing. 

Up to five sequences may be linked to form a series. The carry-out is also suitable as an  ALTERNATIVE EXERCISE combined with a limbering-up exercise like the  BAGGY TONGUE .

Characteristics:

Crude motor, static, stretching, mobility, tone increase.

Remarks:

There may occur complications in case of an existing Gothic Palate.

Instructions:

The stretching factor is especially aiming towards the sublingual region and, secondarily, the tongue lifters and mandibula deflectors( Anatomy 1.3, Area III, Page 22). There may occur a painfulness in this region being explained to the patient as “stiff muscles”. In case of difficulties to gain a CAT’S HUNCHED BACK respectively a pro and down tendency of the tongue belly the exercise  HOLD A STICK may be included.

As mentioned above the here discussed exercise differs from the  SUCK-AND-PULL. As with the later the emphasis lies on the stretching of the sublingual textures, here, the tongue dorsum has to be held against the palate with a static component in the carry-out, meaning a remarkable increase of the muscle actions in comparison. Monitoring is implied through the order “let go!” while the patient lets the tongue sink down for a short moment. If this is accompanied by a popping sound it was incorrectly sucked fast.

In contrary to this the lateral stretch of the tongue tissue may be worked out with the  CHEEKBUMP.

This exercise is frequently mentioned in literature as the DANGLING TONGUE.
